ü    Duly accomplished application forms (for just married Phil women,
 pls write your former last name as new "MIDDLE NAME"),
forms can be downloaded from

ü    3 Royal blue passport size pictures (please do not hold)

ü    595 Norwegian Kroner in cash

ü    Photocopy of the first and last page of old passport

ü    Return envelope to be obtained from the desk

Further to email below, please be informed that the nearest photo shop to the venue
 (Skt. Annæ Kirke) is at CHRISHOLM Fotoshop inside the
Copenhagen Central St. (opposite of McDonalds).. Applicants may wish
 to have their Machine Readable Passport (royal blue background) photos taken there.
